The
Health Centre operated at U.W.I. will be the first
line for receiving medical attention. |

Medical
Associates Private Hospital, Auzonville Medical Centre
and St. Augustine Private Hospital, otherwise referred
to as Private Medical Facilities (PMF), are the Providers
of choice at which Credit Facilities have been established
for services rendered after 8 p.m. or on referral
|

The
U.W.I. Health Centre would refer students to any PMF
for further medical attention needed. |
| 4. |
In
order to access the Credit Facility, students referred
for medical attention at a PMF, would be required
to produce their U.W.I. ID card and sign the Guarantor
Form |
| 5. |
Students
accessing service at a PMF before 8p.m. without a
referral, and those who do not produce their U.W.I.
ID card, before being discharged from the PMF would
be required to pay the full cost of service and make
a normal medical claim. |
| 6. |
Emergency
cases would be allowed access at PMF regardless of
time. |
| 7. |
The
student will ultimately be responsible for any differences
in cost between the cost of the services rendered
by the PMF and the cost of the Plan Benefits to which
he/she is entitled. |
| 8. |
Claims
for non Credit Facilities will be on a reimbursement
basis, i.e. students would
I. Pay the full cost of the medical services received
II. Have the regular Caribbean Home Group Health Claim
Form completed
III. Submit these along with originals of the relevant
receipts and bills to the University for transmission
to Caribbean Home.
IV. The University would forward these claims to Caribbean
Home for adjudication and re-imbursement to the student
as otherwise agreed to or advised in writing
